Is the New Injury Severity Score (NISS) a better outcome predictor than the Injury Severity Score (ISS) in patients with musculoskeletal injuries: A retrospective analysis?

Purpose: The Injury Severity Score (ISS) is used to predict outcome after trauma. However, it criticised because of flaws in its calculation injury severity. New (NISS) was proposed as an alternative. studies are conflicted on which better. We compared both scales predicting surgery, multiple surgeries, preoperative blood transfusion, hospital stay length and mortality patients with orthopaedic injuries. Method: A retrospective cohort study that the hospital's trauma database. Patients’ data were extracted, parameters noted. ISS NISS calculated for each patient. dichotomised into discrepant non-discrepant if scores different or same, respectively. receiver operator characteristic (ROC) curve generated parameter, area under (AUC) between two scoring systems. Results: Four hundred forty-seven (447) participated this study. participants’ average age 34.78 years (SD = 18.67), mean score 8.5 5.9), while 9.4 6.6). exceeded (discrepant) 82 subjects (18.3%), same (non-discrepant) 365 (81.7%). outperformed surgeries length, better predicts rate. Both performed similarly surgical intervention transfusion. Conclusion: there insufficient evidence replace NISS.
